HFH Criminal Records Bureau applications
Having your CRB Self Declaration form checked.
Identification documentation you MUST provide when having your application form checked and verified by a HFH Child Welfare Officer.
As a CRB ‘Applicant’, and in accordance with current governing regulations, you MUST provide certain documentation when seeking verification of your application by a HFH Child Welfare officer.
The HFH Child Welfare Officer MUST check AT LEAST 3 f'orms of evidence'.
There are varying types of ‘forms of evidence’ which you could possibly provide and therefore these have conveniently been categorised into different GROUPS, depending on the information they are able to provide to the HFH Child Welfare officer.
YOU MUST PROVIDE AT LEAST 3 FORMS OF EVIDENCE.
Ideally, as an ‘Applicant’ you need to provide 3 GROUP ONE documents.
GROUP ONE DOCUMENTS
1. Valid passport (any nationally).
2. UK Driving Licence (either photocard or paper).
3. Original UK Birth Certificate.
4. Valid photo identity card (EU Countries only).
5. UK firearms Licence.
However, as an alternative, an ‘Applicant’ can provide a total of 3 documents, 1 of which is from GROUP ONE
Or
If no GROUP ONE documents are available, a total of 5 documents from GROUP TWO.
GROUP TWO DOCUMENTS
1. Marriage certificate.
2. Non-original UK Birth Certificate (issued 12 months after birth. Full or short form acceptable.
3. P45/60 Statement (Documents should have been issued within the past 12 months).
4. Bank or Building Society Statement or Utility Bill e.g. electricity, gas, water, telephone, mobile phone contract (Documentation should be less than three months old).
5. Valid TV Licence.
6. Credit Card Statement (Documentation should be less than three months old).
7. Mortgage Statement/Tenancy Agreement (Documents should have been issued within the past 12 months).
8. Valid insurance certificate.
9. Correspondence or a document from the: Benefits Agency, Employment Service, Inland Revenue or Local Authority (Documentation should be less than three months old).
10. Financial Statement, e.g. pension, endowment, ISA (Documentation should be less than three months old).
11. Valid vehicle registration document.
12. Court commons.
13. Valid NHS card.
14. Addressed payslip (Documentation should be less than three months old).
15. National Insurance number card.
16. Exam certificate, e.g. GCSE, NVQ.
17. Child Benefit book (Documents should have been issued within the past 12 months).
18. Certificate of British nationality.
19. Work permit/visa (Documents should have been issued within the past 12 months).
If you are providing as evidence of identification any Utility Bills, then you must ALSO complete the RFL CRB Evidence form. This will be supplied by the HFH Child Welfare Officer.
There have been cases whereby CRB forms have had to be returned because the ‘Applicant’ has been a mistake. This obviously leads to a delay in the form being processed.
To avoid these delays happening to you, CHECK your application form before meeting with a HFH Child Welfare Officer.
Often, it’s the result of simple errors which can be spotted by having a second check of the form.
When completing your CRB application ensure that your:
*Passport/Driving Licence numbers are entered correctly.
*valid ‘from/issue’ dates are included where applicable.
Most common ‘Applicants’ mistakes are:
Using blue ink – only BLACK ink should be used.
Using correction fluid – if an error is made, a line should be put through it and the correct information clearly written to the right of the crossed out information.
‘Option’ boxes being ticked – they should be marked with a X.
Signatures going outside the box.
Is a section is not relevant, please leave it blank – do NOT write N/A or cross it out.
Do NOT staple anything to the application form – continuation sheets should be attached using a paperclip.
The ‘Applicant’ should NOT complete X, Y or Z.
Section C Q22 – only enter surnames, do NOT enter forenames.
Please leave section E & F blank.
Not providing a continuous 5 year address history – if you have not lived in your current address for 5 years, you MUST write down any other addresses you have lived at in the past 5 years, and the relevant dates.
Not providing middle name(s) in the forename(s) box.
Hopefully the above information will assistant all HFH ‘Applicants’ when completing and submitting, for verification to HFH Child Welfare Officer, their completed CRB Self Declaration application forms.
